2-3) is necessary for the successful completion of the contract. The submission of personal data is voluntary however a failure to submit personal data results in failure to complete the contract with the data controller.

3) shall not be disclosed to any other than the data subject, controller, processor and persons who, under the direct authority of the controller or processor, are authorised to process personal data;
4) as pertains to submitted personal data, the client has the right to:

4-1) request access to their personal data;
4-2) request the rectification, erasure or limitation of personal data processing;
4-3) object to data processing;
4-4) transfer their personal data;

5) if the data is processed based on a signed agreement contained in the contract, the agreement is voluntary and the client has the right to withdraw their consent at any time.
6) 6) in the case of the data controller’s violation or infringement of the Regulation on the Processing of Personal Data, the client is entitled to lodge a complaint with a supervisory authority. The complaint should be addressed to Biuro Generalnego Inspektora Ochrony Danych Osobowych. ul. Stawki 2, 00-193 Warszawa, or submitted via the Electronic Inbox (http://www.giodo.gov.pl/493/).
